Medical College Application for Admission
Complete this form if you are an international student applying for the Doctor of Medicine (MD).
This form is used for submittal of the application for enrollment at the University.
Instructions:
• Complete all sections of the Application for Admission form.
• Read the relevant information for the course you wish to apply for and ensure that you provide any
additional information required.
• Read all Sections of this form carefully to ensure your application meets the documentary
requirements.
Submit the completed Application for Admission form with all supporting documentation.
Essential supporting documentation:
All applications must include clear copies of the following documents:
* The copies of your academic record from your current or recent studies (or University if you are
a transfer or visiting student) and its supplement (official transcript of grades); For postgraduate
courses – copies of official transcripts and proof of completion of undergraduate courses
completed.
* A copy of your passport.
* If you reside overseas and/or your education qualifications are held from countries where
English is not the main language, you must provide evidence of proficiency in the English
language.
* Application fee – The application fee for all degree applicants is US $100 (please note that this
fee is non-refundable and application cannot be reviewed until the application fee has been
processed. The application fee can be waived if student covers all third-party costs incurred by
the application process. In certain cases, visa processing fees can be charged separately).
* All documents that the Admissions department requires must be translated in English.

and for other administrative functions within the University. The personal information you
provide will not be made available to any other person or organization outside of the University
or for any other purpose without your consent or where authorized by law, and will be disposed
of in accordance with Government regulation. You may access your personal information to
ensure that it is not inaccurate, irrelevant to the purposes for which it was collected.
